Course Substitution Request Checklist
Course Substitution Request ChecklistDistrict policy requires that all requests for course substitutions based upon disability be reviewed by the ADA Academic Council.
DRS Managers should assist the student in compiling the information listed below and forward as ONE document to Compliance.
- General Requirements
- Must be a current MCCCD student and
- Registered with Disability Services with an active course accommodation plan
- Letter of support from current Disability Resources & Services Manager
- Documentation of disability by a qualified medical professional, to include the following: (only information that has already been submitted to the DRS may accompany a course substitution request. Please submit only the most current documentation - within FIVE years.)
- Clear disability diagnosis with description of symptoms
- Current level of functioning
- Adult-normed standardized tests
- Substantial limitation determined
- Impact on learning determined
- Letter of request from student, to include the following:
- History of course attempts
- History of academic accommodations and tutoring services
- Current program of study
- Requested course to be substituted (see #4)
- Unofficial transcripts (must support a history of course completion or withdrawal)
- High School (if applicable)
- College(s)
